Prior to your appointment, we encourage you to review the preparation documents. Please make sure to bring all of your insurance documents and arrive at your appointment early enough to allow for time to check-in. Some procedures will require you to stop taking certain medications prior to your scheduled procedure. Always check with your radiology department for specific instructions regarding current medications.

Fill out this Authorization for Release of Health Information form prior to your request. Once processed, you may pick up images at one of the following locations:
